就是可以直接打印，准备答辩了气管插管时异丙酚－瑞芬太尼合理配伍相关问题探讨摘要：目 的 气管插管是全身麻醉中极其重要的步骤之一，其特点是：历时时程短，对人体刺激强。有人测定，发现其刺激强度大于切皮时的疼痛刺激，若不能合理的抑制由插管引起的神经－内分泌－心血管等一系列反射，可能对病人造成严重的伤害。合理的麻醉方法是抑制这些伤害性刺激的有效手段。但在日常的工作中，常用的麻醉药物和方法不能有效地阻断这一反射，寻求更好更有效的手段是麻醉界奋斗的目标。本研究配伍应用较新型的麻醉药异丙酚和瑞芬太尼，试图寻找一种较为合理的浓度搭配和相关的参数如半数有效量等，以求为麻醉临床工作提供一些有益的数据。 方 法 年龄18～59岁,体重指数在18～25千克/平方米,ASAⅠ-Ⅱ的病人为研究对象。在第一部分中,以系列浓度：2.8、3.2、3.6、4.0、4.5、5.0、5.6μg/ml的异丙酚，对32位病人序贯法靶控输注诱导，用序贯分析法计算其PEC50值;在第二部分研究中,把100位病人随机分为五组,伍用2倍半数有效浓度的异丙酚（2PEC50）的异丙酚，并分别接受6.3、8.0、10.0、12.5、15.6ng/ml的瑞芬太尼，采用效应部位浓度靶控输注诱导，研究异丙酚联用瑞芬太尼无肌松插管的量效关系。并采用概率单位回归分析法计算出瑞芬太尼的REC50、REC95。 结 果 测得PEC50为3.04μg/ml; REC50、REC95分别为7.6ng/ml和17.8ng/ml。靶控输注6.0μg/ml异丙酚和15.6ng/ml瑞芬太尼诱导,基本能提供满意的临床插管条件.但 http://www.txlunwenw.com/article_show.asp?pageid=3447需注意循环系统的安全。 结 论 瑞芬太尼和异丙酚靶控输注诱导，获得的插管条件与靶控浓度相关，理论上6.0μg/ml异丙酚联合17.8ng/ml瑞芬太尼靶控输注诱导,基本能提供满意的临床插管条件.但联用两者靶控输注诱导插管的"最适配伍浓度"还需进一步研究.关键词：气管插管 异丙酚 合理配伍Abstract: Tracheal intubation is one of the most important steps of general anesthesia,It lasts a short period of time but has strong stimulation to body.Somebody had determined that its stimulus intensity was bigger than that of inciding skin, It may result in severe injuries to patients unless restraining trail reflex as nerve- endocrine- cardiovascular system.Rational anaesthetic method is a effective mean to restrain these nociceptive stimulus.In daily work, commonly used anaesthetic drugs and means can't block these reflex efficiently that exploring more effective methods is needed. try to search rational concentration-match and related parameter such as median effective dose by compatible application of propofol and remifentanil,and it may provide some beneficial data for clinical anesthesia.
